How can I submit a participation fee form for Alberta and/or Ontario?

Select ‘Yes’ to the question ‘Are you required to file a participation fee form in Alberta and/or Ontario?’ when creating your annual financial statement filing.
 
In contrast to other filings, after submitting your annual financial statement and completing payment, the confirmation page provides a link to ‘Create participation fee form’. This link displays only if the recipient agency is Alberta and/or Ontario, on the ‘Filing and contact details’ page. Alternatively, simply select ‘Participation fee form’ from the ‘Annual’ sub-menu, under the Continuous Disclosure sub-menu.
